The questionnaire below is optional, but will greatly assist us in determining your overall qualifications for Team Friendship. In other words, if your photo and story doesn't cut it, filling out this questionnaire could really help in obtaining your Legionnaire status. Participation in this questionnaire is HIGHLY ENCOURAGED. As long as your reading level is above a third grader, it should only take you one or two minutes to complete it.
